Each new player should complete and return a copy of the Player
Information Form along with a copy of his birth certificate.
Birth certificates are required by league and tournament officials
for verification of a player's age.
The Player Information Form is needed to complete scouting forms for
tournaments and recruiters as well as maintain rosters for upcoming
tournaments and the Web site. Complete the Player Information
form and either fax to the number below or mail to the address
below.
