All new and returning players are required to
complete a Player Information Form at the beginning of the season.
New players must also send a copy of their birth certificate in
addition to the completed Player Information Form. See
New Players page.
This information is needed to complete scouting forms for
tournaments and recruiters as well as maintain rosters for upcoming
tournaments and the Web site. Complete the Player Information
form and either fax to the number below or mail to the address
